These providers cover diagnostic and preventive services in full and charge minimal patient co-pays for other services. There are no deductibles, no maximums and no waiting periods for any in-network treatment. Out-of-pocket expenses are low and coverage is extensive.

CapDent Plus Select allows managed care members to use non-participating dentists at any time. Patients who seek out-of-network care are reimbursed according to the Schedule of Benefits for covered procedures. While some deductibles and maximums may apply, "freedom of choice" means you get the care you want at a cost you can afford.

In this point-of-service program, you and your covered family members select a dentist from the CapDent Directory of Participating Providers and receive all treatment from that dentist. Click here to see the list of participating providers in your area. Some services are rendered without any cost, while others have a minimal copayment that you pay directly to the dentist. For specialty care, members are treated by a Healthplex PPO specialist. If you would like to go to your own dentist, Healthplex will reimburse you.
